Getting started is as easy as 1, 2, 3!
At Kaiser Permanente, our members are at the center of everything we do. We’re focused on your health and wellness. And to encourage you to share our focus, the State of Maryland wellness program gives you the opportunity to earn wellness rewards.
Complete the three healthy activities listed below and you’ll receive your wellness reward: $0 copays for all primary care physician visits.
- Sign onto kp.org/engage.
- Designate a primary care physician at enrollment.
- Complete a Total Health Assessment.
NOTE: The State of Maryland wellness program applies to employed subscribers and their spouses and non-Medicare retirees and their spouses.
You may take the health assessment as often as you like, but you can only receive the reward for it, if available, once during the reward period.
Wellness programs and rewards are separate services that are not health plan benefits and may be discontinued at any time.
